Here is the Mental Hazard Profile. Alan Shapiro, Ph.D. wrote a great book that examines “Golf’s Mental Hazards” – Overcome them and put an end to the Self-Destructive Round. The book is recommended to all golf professionals, by the PGA, so I’m sure sharing this is no problem.
Here are the 8 pages on the Mental Hazard Assessment Scale questions, answers and ratings to identify what needs to changed. It is straightforward and self administered. This test is recommended to be taken quickly from a gut feel perspective. Once completed it provides insight into hazards give you little trouble and which are responsible for a self-destructive round. It is worth the few minutes to do and provides great insight.
Other materials that I have used in my coaching sessions are “CoachRick” specific and done Prior to becoming a New Rules Golf Coach. Feel free to use these as templates
